Dear Ian,

 We discussed and I raised with John Parkes who is responsible for the SITREP Return to Region.

John P is wholly accountable for the return and he advises that the cancellations data is not from pas due to the lack of robustness of the data. This has not been hidden, and our Audit Commission return flags up that John P is responsible for the return.

I understand that further work is being undertaken to bottom out the pas report-as other trusts across the former SW Thames area do not appear to have the same problems we have.

Wendy needs to be assured by you that John P takes responsibility for the return.

I think you need aware of issue that I need to draw your attention which concerns the integrity of the information that the Trust is currently sending the Regional Office.

A member of the Information Staff has raised concerns about the Trust's reporting of "Cancellation of Elective Surgery".  Kelly Goulding who is a grade 5 A&C member of the Information team has this week while Jenny Jones is away, been entering the weekly SITREP report information.  For the period 1/10 to 7/10 among other required information she entered on to the report that we had 28 patient cancellations.  However, before she sent the report off she had a telephone conversation with John Parkes who told her that she was to report zero in this category, as this was an area in which the Trust was being closely monitored. Kelly checked the previous weeks figures SITREP report against PAS and found that although Jenny Jones had reported zero for cancelled patients PAS in fact showed that the Trust had 42 cancelled patients.  When this point was raised within the Information Department, Tracey Dean said that Jenny Jones had told her that the figure had similarly  been reduced to zero on John's instructions.

You will appreciate that this potential mis-reporting of these figures has caused some concern within the Directorate as staff are concerned that this may be an issue that will criticised at a later date and that they themselves may be accused of mis-reporting the position. I would be grateful if we could discus this issue at our meeting this afternoon.
